Note: Please return this form along with the registration fee of RM30.00 to Euphony Musical S/B Ikano by October 6th, 2010. Contact person: Lay Peng / Maggie Telephone Number: 012-300 7177 / 4291 7177. Please enclose a copy of valid certification of the contestants if any. The song composed by the contestants shall also be enclosed if any. Please bring along your own CD or Audio Disc for accompaniment if needed, Euphony Musical S/B will not be responsibility for any loses, damage or stolen of your musical instrument(s) and belongings.

Rules and Regulations:

Event Summary:

1)      The Euphony Musical S/B Instrument Performance Contest 2010, which is sponsored by Euphony Musical S/B

2)      It will start on Sunday from 10:00 AM (Registration) to 3:00 PM (Prize Announcement).

3)      Registration Fee: RM30.00 (One person)

4)      We reserve the right to change the Rules at any time without previous announcement.

Prizes:

1)      There are 3 prizes for each category: Grand Winner, First Runner-up and Second Runner-up      (Category A 5 – 12 years old), Grand Winner, First Runner-up and Second Runner-up (Category B 13 – 18 years old)

2)      Grand Winner will get RM 500 worth of a musical instrument sold in Euphony Musical S/B, First Runner-up will get RM 300 worth of a musical instrument sold in Euphony Musical S/B and Second Runner-up will get a RM 100 worth of a musical instrument sold in Euphony Musical S/B.

3)      Any winner can and allowed to top-up in any case he / she wanted a musical instrument which its value is more than his / her prize value.

Eligible Participants:

1)      The contest is open to participants who are age between 5 to 18 years old.

2)      All participants must be Malaysian.

3)      Members of immediate family to any Euphony Musical S/B employees or employers are not allowed to participate in this contest.

4)      All participants must own their musical instrument or purchased from Euphony Musical S/B only.

Contest Categories:

1)      Categories are divided into 2, Category A 5 – 12 years old, Category B 13 – 18 years old.

2)      Eligible musical instruments: Strings including violin, viola, cello, double bass, piano, guitar, Chinese instruments such as Gu Zheng, Er Hu, Pi Pa, woodwind such as flute, Chinese flute, saxophones, clarinets, harmonicas, percussion such as drum sets, conga, brass such as trumpet, trombone, digital such as keyboard and organ.

3)      All participants are required to bring their own accompaniment music compact disc (CD) if needed for their performance, evaluation given are solely based on overall performance only.

4)      All performances are judged by professionals relevant to music field, music teacher or artists.

Eligible Entries:

1)      Participant is required to fill out the Euphony Musical S/B Musical Instrument Performance Contest Form, which can be obtained at any Euphony Musical branch, and to be submitted by the day before the contest, which is on 9/10/10 Saturday.

2)      All late submission of contest forms will not be entertained.

3)      Euphony Musical S/B reserved the right to cancel or disqualified the participant if found the contest form is not complete or lack of details of the participant.

4)      All participant are required to report for registration at the registration counter at the Main Concorde Area by 10:00 AM, late comers will not be entertained.

5)      Participants are required to bring their own make-up kits and dresses for their performance, dressing rooms will be provided.
